WHAT IS HOME FIRST?
Home First is a philosophy to help frail patients get out of hospital and back into their homes as soon as possible. It's meant for patients who have completed acute care treatment and no longer need 24-hour attention in hospital, patients who can heal safely at home with the right kinds of support.


Home First connects you to that support with enhanced home care services for up to 60 days.



HOW IT WORKS
Home First starts with a plan of care designed to suit each patient's needs. CCAC work with you and your family or caregivers along with hospital staff, physicians and local health networks to create a detailed picture of the support that will best help you make the difficult transition from hospital to home. They connect you with dedicated nurses, personal care assistants, physiotherapists and other quality professionals, and can help you get the medical equipment you need.


The family member is partnered with a CCAC care Coordinator to advocate on their behalf and put your plan of care into action. Care Coordinators specialize in identifying and accessing community supports designed to help patients maintain their independence and quality of life at home.



HOW IT HELPS
Home First helps the client heal. Patients who no longer need the acute care of a hospital setting often heal better at home, if only they have the right support. The Care Coordinator can help access the network of excellent home care and community services available in the community through CCAC.


Home First helps patients avoid long term care homes. Most patients who access services through the Home First philosophy are able to remain in their homes after the transition program runs its course.


Home First gives families time at home to make informed decisions. After leaving hospital, some find that living independently is no longer a realistic option. The Home First transition period offers a respite at home and a chance for patients, families and caregivers to consider all their long
